This Privacy Policy describes how we collect, use, protect, and share information when you visit rozenbergdentalnyc.com (the “Site”) or contact us through it.
This Policy governs information collected through the Site. Health information we create or receive about you as a patient is protected health information (“PHI”) and is governed by our HIPAA Notice of Privacy Practices, described below.
Rozenberg Dental NYC is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and complies with the HIPAA Privacy Rule, Security Rule, and the Breach Notification Rule under the HITECH Act. As our patient, you have rights under HIPAA, including the right to:
We apply the “minimum necessary” standard to internal uses of PHI, train our workforce on privacy and security, and require vendors that handle PHI on our behalf to sign Business Associate Agreements.
Please note: our website forms are intended for scheduling and general inquiries only. Please do not submit detailed medical or dental history, insurance identifiers, or other sensitive health information through the Site. That information should be shared directly with our team by phone or in person.
Information you provide. When you submit an inquiry or request an appointment, we collect the information you choose to share — typically your name, phone number, email address, the service you are interested in, and an optional message.
Information collected automatically. Like most websites, our hosting infrastructure records standard technical data such as IP address, browser and device type, pages viewed, referring pages, and timestamps. This data is used for security, troubleshooting, and aggregate site analytics.
Cookies and tracking technologies. We keep tracking on this Site to a minimum. We do not place third-party advertising pixels or ad-network trackers on pages of the Site, consistent with guidance from the HHS Office for Civil Rights regarding online tracking technologies for HIPAA-regulated entities. Embedded third-party services (such as Google Maps and Google Fonts, described in Section 9) may set their own cookies subject to their own policies.
By providing your phone number and submitting an inquiry, you consent to be contacted by our office by phone call and/or SMS text message regarding your inquiry and appointment-related matters, in accordance with the Telephone Consumer Protection Act (TCPA). Consent to receive text messages is not a condition of receiving care.
We do not sell your personal information. We share information only with:
We maintain reasonable administrative, technical, and physical safeguards designed to protect personal information, consistent with the New York SHIELD Act (N.Y. General Business Law § 899-bb) and, for PHI, the HIPAA Security Rule. These safeguards include encryption of Site traffic (HTTPS/TLS), access controls, and workforce privacy and security training. In the event of a breach of private information as defined by New York law, we will provide notification in accordance with N.Y. General Business Law § 899-aa and, where PHI is involved, the HIPAA Breach Notification Rule.
We retain website inquiry information for as long as needed to respond to you and maintain our records, and we retain patient records for at least the period required by New York State law and dental board regulations. When information is no longer required, it is disposed of securely.
The Site embeds Google Maps (to display our location) and uses Google Fonts. The Site also links to third-party platforms such as Google Reviews and Instagram. These services are operated by third parties under their own privacy policies, which we encourage you to review. We are not responsible for the privacy practices of third-party sites.
